Important Notice:-

Signature should match as per the passport in each and every documents and forms. If the signs do not match, applications will be rejected.

 

SERVICE - CHANGE OF NAME

Fees : HK $ 595/-: For ordinary Passport booklet containing 36 pages

a) BLS service charges: HKD 47


HK $ 788/-: For Jumbo Passport booklet containing 60 pages

a) BLS service charges: HKD 47
Documents required :

Category 1 : Change of name

  • An applicant applying for re-issue of passport for incorporation of the name of his/her spouse
  • A woman applying for change of name/surname in existing passport due to marriage
  • Divorcees applying for change of name or for deletion of spouse's name in existing passport
  • Re-married applicants applying for change of name/spouse's name in passport

Category 2: All other cases not covered in Category 1 including minor change in name, change/addition of surname and married woman wishing to revert to maiden name when not divorced or widowed

Category 3: To change personal particulars like change of address, appearance, place of birth, date of birth

Requirements for Category 1

  • Applicants are required to fill online application form at https://embassy.passportindia.gov.in and carry a printout of the filled in application form
  • Personal presence of the applicant is required.
  • Three recent passport-size photographs of 2 inch x 2 inch, full face & ears visible and white/light background
  • Original and self-attested photocopy (first two and last two pages) of passport
  • Original and self-attested photocopy of Hong Kong/Macau ID/valid visa
  • Fee of HKD 596 for 36 pages and HKD 796 for 60 pages passport
  • Annexure E Form

  • Applicants are required to fill online application form at https://embassy.passportindia.gov.in and carry a printout of the filled in application form.
  • Personal presence of the applicant is required.
  • Three recent passport-size photographs of 2 inch x 2 inch, full face & ears visible and white/light background
  • Original and self-attested photocopy (first two and last two pages) of passport
  • Original and self-attested photocopy of Hong Kong/Macau ID/valid visa
  • Gazette notification published in the Indian Union/State Official Gazette; OR Clippings of the two local (in India and Hong Kong/Macau) newspapers OR two public/school documents issued in the desired name
  • Fee of HKD 596 for 36 pages and HKD 796 for 60 pages passport

Requirements for Category 3

  • Applicants are required to fill online application form at https://embassy.passportindia.gov.in and carry a printout of the filled in application form
  • Personal presence of the applicant is required
  • Three recent passport-size photographs of 2 inch x 2 inch, full face & ears visible and white/light background
  • Original and self-attested photocopy (first two and last two pages) of passport
  • Original and self-attested photocopy of Hong Kong/Macau ID/valid visa
  • Original and self-attested photocopy of supporting documents for change of personal particulars as per the case

Fee of HKD 596 for 36 pages and HKD 796 for 60 pages passport

1. Applicants parents who are born before Partition (1947) in Karachi, Sind, Hyderabad, etc (places which are in Pakistan now), please write UNDIVIDED INDIA in the country category. Otherwise, forms will not be accepted.


2. If applicants/ parents of applicants/ spouse are not Permanent HKID holders, have to also submit the Current Visa page copy (SELF ATTESTED)


Note : Applicant must apply in person for the above service.


All copies attached to the application must be self attested.
