Relationship Certificate (Bilingual) - One person

Documents Required:

Duly filled Affidavit in duplicate Click here. (Form Filling services are available in the BLS center)
Copy (ies) of all document(s) related to support of the relationships i.e. passport, notarized affidavit (for other than spouse, children & parents of applicant) duly attested by the Home Department of the concerned State Government and Ministry of External Affairs, public documents etc.
In case an applicant asks for Relationship Certificate for Spouse, their names should be entered in the Passports (if both passports are issued after marriage).
If the spouse's name is endorsed on one of the passports, but the other passport was issued after marriage, Copy of duly attested Marriage Certificate must be submitted. The original Marriage Certificate must be presented for verification.
If neither the husband nor the wife has the spouse's name endorsed on their passports, and the passports were issued after marriage, one of them must have the spouse's name added to their passport. Copy of duly attested Marriage Certificate must be submitted, and the original Marriage Certificate must be presented for verification.
If an applicant is applying for a relationship certificate for their adult son or daughter, the spouse's name must be endorsed in both the passports.

Original Passport of the applicant.
