Relationship Certificate (Bilingual) - One person

Documents Required:

Duly filled Affidavit in duplicate Click here. (Form Filling services are available in the BLS center)
Copy (ies) of all document(s) related to support of the relationships i.e. passport, notarized affidavit (for other than spouse, children & parents of applicant) duly attested by the Home Department of the concerned State Government and Ministry of External Affairs, public documents etc.
In case an applicant asks for Relationship Certificate for Spouse, their names should be entered in the Passports (if passport issued after marriage).
If spouse name is not endorsed in Passport, need to submit duly attested Marriage Certificate.
Copy of Civil ID Card of applicant. (Photocopy services are available at the BLS center)
Original Passport of the applicant.

Applicant has to come in person.
The Applicant must produce Passport, Civil ID or any other related document in original for verification.
The proposed Certificate, Undertaking etc. should be type written only.
